Role of the Care & After Team

All young people who have been looked after by Bristol for more than 13 weeks between their 14th and 16th birthdays (excluding short respite care placements) and who are still looked after on their 16th birthday will receive support from the Care & After Team from age 18-21 (or up to 24 for young people in full-time education, or 25 if young person has a disability).

The Care and After Team will give the following help to young people:

1. Financial advice and financial assistance when appropriate.

2. Advice and support with education, training and employment.

3. Advice and support with health, and family and social relationships

4. Advice and support with securing and maintaining suitable accommodation

5. Respond to the needs of young people who experience difficulties and any crises during their journey to adulthood.

6. Access to Representation and Complaints procedure.

7. Provide opportunities to give feedback on the service and involve them in the decision making process, not only in relation to their own future but also developing services for the future.
